1) No it’s not. There are only 18 symbols, with each symbol appearing once on each level; you can see this if you pause the episode and go frame by frame. The Doctor even by the most conservative count has had more than 18 companions.
2) You left out a fuck ton of companions on that list. Even if you’re sticking with just the TV show. Starting with Steven Taylor, Dodo Chaplet, Katarina, Victoria Waterfield, Brigadier Lethbridge-Stewart, Captain Mike Yates, Sergeant John Benton, Vislor Turlough, and Adam Mitchell. And I suspect I’m still missing some. Not to mention all of the companions from the Extended Universe, which brings the total number closer to a hundred.

In the Torchwood episode “Captain Jack Harkness,” Torchwood leader Jack is taken back to 1941, and for the first time meets the man he took his name from. The two Jacks are inexplicably attracted to each other, and because Torchwood Jack knows that the historical Jack will die the next day, he takes the risk of dancing with him in front of the shocked party-goers.
